Importance: Given the importance of apathy for stroke, we felt it was time to scrutinize the psychometric properties of the commonly used Starkstein Apathy Scale for this purpose.Objectives: The objectives were to: estimate the extent to which the SAS items fit a hierarchical continuum of the Rasch Model; and estimate the strength of the relationships between the Rasch analyzed SAS and converging constructs related to stroke outcomes.Methods: Data was from a clinical trial of a community-based intervention targeting participation. A total (...) of 857 SAS questionnaires were completed by 238 people with stroke from up to 5 time points. SAS has 14 items, rated on a 4-point scale with higher values indicating more apathy. Psychometric properties were tested using Rasch partial-credit model, correlation, and regression. Items were rescored so higher scores are interpreted as lower apathy levels.Results: Rasch analysis indicated that the response options were disordered for 8/14 items, pointing to unreliability in the interpretation of the response options; they were consequently reduced from 4 to 3. Only 9/14 items fit the Rasch model and therefore suitable for creating a total score. The new rSAS was deemed unidimensional. Apathy was correlated weakly with anxiety/depression and uncorrelated with physical capacity. Regression showed that the effect of apathy on participation and health perception was similar for rSAS/SAS versions: R2 participation measures ranged from 0.11 to 0.29; R2 for health perception was ∼0.25. When placed on the same scale, rSAS value was 6.5 units lower than SAS value with minimal floor/ceiling effects. Estimated change over time was identical which was not substantial but greater than expected assuming no change.Conclusion: The retained items of the rSAS targeted domains of behaviors more than beliefs and results support the rSAS as a robust measure of apathy in people with chronic stroke. Three of them are epistemic and three are ethical. In this commentary I consider how service users might relate to values if significant levels of scientific knowledge are required to understand those values. I specifically consider the traditional theoretical virtues discussed by philosophers of science (Psillos, 1999; (...) Solomon, 2001) whereby we might judge scientific concepts based upon their simplicity, consistency or coverage. This raises questions of how democratic approaches should function when expertise is important.There is a general problem of how democratic institutes should function when members lack understanding. For example, most voters in current European democracies have limited understanding of economics. They may understand some broad principles but would not understand how to actually manage an economy on a daily basis. A simple solution to this is that political parties outline a broad framework and then are given consent to run the economy once elected. Our elected representatives or members of an unelected bureaucracy actually then make the "on the ground" decisions about how to run the economy given the broad framework which the political party was voted in on. This hopefully then means that, with varying degrees of success, people who have some understanding of economies are playing a key role. We can certainly ask important questions about this system, such as what level of economic understanding do citizens have and what level they need, who should constitute the unelected bureaucracy and whether the bureaucracy could actually be elected. My simple point is that this is one approach to limiting the decision-making of people who may lack economic expertise. In relation to democratizing psychiatric research, I consider whether Friesen's approach might mean service users require a level of expertise that they typically lack.Friesen outlines how there is a very high level of value decision involved in psychiatry given the gap between data and theories. Friesen thinks service users should be included in decisions over what values to use. I now provide an example of where making good value decisions might require [End Page 135] significant levels of scientific knowledge. I consider a specific type of value applied to a specific area. There has been recent discussion of service users being involved in decision-making over modifying psychiatric diagnoses, whereby they might play a role in deciding what psychiatric diagnoses there are and what their diagnostic criteria should be (Bueter, 2019; Fellowes, 2023; Tekin, 2022). I consider how we can assess psychiatric diagnoses using theoretical virtues. It should be emphasized that this problem may or may not have applicability to other areas of service user involvement, that theoretical virtues are not the only values which are important for assessing psychiatric diagnoses and that scientists might themselves make bad decisions in relation to theoretical virtues. I now outline five theoretical virtues.First, simplicity. There are lots of different ways to understand simplicity but here I consider how simple a system of psychiatric diagnoses (i.e., the Diagnostic and Statistical Manual of Mental Disorders [DSM]) is. We can ask if the system of psychiatric diagnoses is too simple because it contains too few diagnoses or too complicated because it contains too many diagnoses. Simplicity and complexity might also depend on the degree different diagnoses have overlapping symptoms. We can judge a particular psychiatric diagnosis on the degree it adds to simplicity or complexity. Establishing the strength of this theoretical virtue requires knowledge of many different psychiatric diagnoses.Second, coverage. This relates to the degree that all the phenomena that we desire covered are being covered. We can ask whether a system of psychiatric diagnoses is or is not covering all the symptoms that we think need covering by psychiatry. We can judge particular diagnoses on how they contribute to coverage. Assessing this requires knowledge of a vast range of symptoms.Third, embedded within a theoretical network. Decades of feminist research has revealed the dismal labor conditions for women in global supply chains. Given this reality, why does Walmart use women in its supply chain as icons of female empowerment? Combining the Marxist notion of a “spatial fix” with a feminist analysis of symbolic resources, we develop the concept of a “gender fix” to understand a growing field of corporate programs that use women as symbolic resources to restore the image of firms as ethical actors. The gender (...) fix encompasses a wide range of corporate empowerment strategies to capture a broad pattern of action. A second term is necessary to situate Walmart’s particular empowerment strategy within its corporate positioning as leader of a vast supply chain network. We identify Walmart’s particular empowerment strategy as “outsourcing feminism.” Walmart’s use of the outsourcing feminism strategy stems from its commanding position in supply chains, which allows it to extract a symbolic surplus from women business owners from whom it sources products. When Walmart argues that it uplifts these women, its strategy targets the consumer end of the supply chain, emphasizing sourcing practices that promote a racially diverse, selfless, and middle-class femininity. In so doing, Walmart deflects attention from the treatment of women in other segments of its supply chain (e.g., its stores and factories). (shrink)

In response to the need for Holocaust curricula in Latvia, Latvians and Americans worked collaboratively to overcome the historical silence surrounding this event. During their project, Latvian curriculum writers worked with teachers and scholars at the United States Holocaust Memorial Museum. This descriptive analysis of the Latvians' experience with Museum Fellows revealed opportunities to learn from each other the complexities of teaching the Holocaust in a country viewed by some as collaborators and still somewhat anti-Semitic. Findings included depth of (...) guidance, values, and limits of innovative teaching methods, cross-cultural benefits, and continued collaboration. Recommendations for future efforts by post-Communist countries and American partners to develop curriculum on teaching a most complex and contentious aspect of history such as the Holocaust conclude the study. To inform medical education reform efforts, we systematically collected information on the level of arts and humanities engagement in our medical school community. Attitudes regarding incorporating arts and humanities-based teaching methods into medical education and patient care were also assessed. An IRB-approved survey was electronically distributed to all faculty, residents, fellows, and students at our medical school. Questions focused on personal practice of the arts and/or humanities, as well as perceptions of, and experience with formally incorporating these into medical (...) teaching. Of 13,512 community members surveyed, 2,775 responded. A majority of respondents agreed or strongly agreed that medical education and patient care could be "enhanced" by the integration of the arts. There was enthusiastic support for the creation of a formal program in the arts at our medical school. Integration of the arts into medical education may have a role in improving the quality of medical training and would likely be well received by teachers and learners. (shrink)

The Term ‘liberty’ invokes such universal respect that most modern political economists and moralists endeavour to find a conspicuous place for it somewhere in their systems or prescriptions. But in view of the innumerable senses of this term an insistence on some kind of definition prior to any discussion seems to be justified. For our present purposes attention to two particularly conflicting interpretations will be sufficient. These are sometimes called the ‘negative’ and the ‘positive’ notions of Liberty. According to the (...) ‘negative’ notion, my own liberty implies the reduction to a minimum of the deliberate interference of other human beings within the area in which I wish to act. Conversely the absence of liberty, or coercion, is regarded as undesirable because it amounts to the prevention by other persons of my doing what I want. On the other hand, the ‘positive’ sense of the word ‘liberty’ consists in the attainment of self-mastery, or, in other words, the release from the domination of ‘adverse’ influences. This ‘slavery’ from which men ‘liberate’ themselves is variously described to include slavery to ‘nature’, to ‘unbridled passions’, to ‘irrational impulses’, or simply slavery to one's ‘lower nature’. ‘Positive’ liberty is then identified with ‘self-realisation’ or an awakening into a conscious state of rationality. The fact that it is contended that such a state can often be attained only by the interference of other ‘rational’ persons who ‘liberate’ their fellow beings from their ‘irrationality’, brings this interpretation of liberty into open and striking conflict with liberty in the ‘negative’ sense. The purpose of this article is to elaborate the many literary allusions that Simone Weil used in her ultimate work: L' Enracinement, translated as The Need for Roots, to achieve her goal of encouraging her fellow countrymen to create a new postwar society. Understanding how she used the riches of the French and Western Literary Cannon, less easily grasped by those not educated in the French Education system, enriches the understanding of Weil's purpose and skill in writing on many levels, (...) simultaneously for different target audiences. Underlying her stress on the need for truth and honesty about a county's past and present, with discernable respect for every person, is her foundational belief in the spiritual destiny of every human being. Examining her literary allusions in detail to show her clever subversion of traditionally accepted interpretations brings a new dimension to Simone Weil studies, while underlining the relevance of this essay to contemporary dilemmas. In the late nineteenth and early twentieth century the name of Adam Smith was popularly associated with the sort of ‘laissez faire’ policy that is expounded with all the fervour of a religious faith. Smith, so the story ran, in his eagerness to combat the excessive mercantilist government intervention of his day, had resorted to supra-natural claims in his general onslaught against central control and planning by governments. Such intervention was ‘unnatural’ and conflicted with Deistic Design. Only through private actions (...) could mankind reach satisfying and ‘natural’ fulfillment. Action through private self-interest was twice blessed; it. blessed him who profited and his fellows who were ‘profited from’. True there was a kind of central planning that was of supreme importance; but it had a divine origin and it worked not through governments but through individual free trade. Private actions in fact were directed by an Invisible Hand which brought society into a grand and spontaneous Natural Harmony. (shrink)

How does the experience of busyness impact democratic political life? My hunch is that those reading this essay might very well offer the following answer: busyness means that we relegate political activities to the bottom of a long and sometimes tedious laundry list of “things to get done.” In fact, many of us no longer even bother to include the basic activities of citizenship –getting informed about the issues, deliberating with our peers about matters of common concern, attending a political (...) meeting, or even voting —on the list in the first place. Like this author, many of you probably feel somewhat guilty about this as well, and thus try to compensate in some way or another… At worst, busyness generates political disinterest and apathy: many of our fellow citizens openly describe the most fundamental form of democratic participation, the vote, as “a waste of time.” At best, it seems to privilege an acceleration of political activity: we seek speedy and rapidly consummated types of involvement that do not unduly add to the enormous time pressures we already feel. Unfortunately, it remains unclear whether even such high-speed forms of citizen involvement are normatively satisfactory. Can liberal democracy flourish when a growing number of us avoid the responsibilities of citizenship altogether, while even those of us who try to remain politically involved insist that they be dealt with quickly and painlessly? In this essay, I hope at the very least to deepen our understanding of this quagmire. If I am not mistaken, it provides a useful starting point for examining one of the core challenges of contemporary political life: how can we make sure that busyness and citizenship coexist in a potentially fruitful, though by no means necessarily harmonious, relationship? This article describes the benefits of including institutional leadership in a faculty fellowship program where faculty were tasked with implementing a curricular innovation at their home institution. These benefits included: serving as an ally, advocate, and defender for the faculty fellow; seeing the bigger picture and how the fellowship can be leveraged to benefit the institution in other ways; and assisting to ensure the fellowship project will be ongoing at their institution.

William Ernest Johnson was a renowned British logician and economist, and also a fellow of King's College, Cambridge. Originally published in 1921, this book forms the first of a three-volume series by Johnson relating to 'the whole field of logic as ordinarily understood'. The series is widely regarded as Johnson's greatest achievement, making a significant contribution to the tradition of philosophical logic. Adequate interpretation of this topic requires understanding the traumas inflicted upon Africans sold into slavery, their existential predicaments before and (...) after emancipation, and the ongoing plight of their descendants whose psyches have been indelibly scarred by this traumatic history. Johnson avails himself of the resources of philosophical theology, psychoanalytic theory, and literary criticism to argue his thesis that African American religion expresses a tragic vision of reality. Besides providing an illuminating analysis of the traumatic experience underlying this religious vision, Johnson presents a compelling constructive proposal as to the broader implications of his thesis for Christian theology at large.As a student of David Tracy and Langdon Gilkey, Johnson sees the necessity for rigorous philosophical reflection in theology. Because the spirituality of the black church stands in some real tension with the traditional theological categories inherited from Europe, Johnson proceeds with “a phenomenological orientation grounded in a philosophical hermeneutical approach” that allows him to excavate the traumatic experiences informing this spirituality (4). The tragic is not the encounter with meaninglessness per se but “the always already aesthetic representation of the encounter, which preserves its terrible truth in a transfigured vision of reality” (6). This vision affirms life while facilitating “the recognition and embrace of the darker, more fundamental truths of life” (7).Cognizant that other theologians have declared the incompatibility of Christian faith with a tragic vision of reality, Johnson believes otherwise. He insists that the tragic is fundamental to human experience as such, although it is frequently evaded by strategies of foreclosure—including those of Christian theology—that falsify the experience. Yet the distinctive thing about the black religious experience is the way African Americans made the Christian faith [End Page 215] their own by transforming it at a deep spiritual level through their tragic vision. For many, the study of literature is still an aesthetic beauty pageant in which “great books” like Laurence Sterne’s Tristram Shandy (1767) or Marie de la Vergne de La Fayette’s La princesse de Clèves (1678) are paraded across the stage in a contest to seduce the Western intelligentsia. 1 The postmodern student of literature may not have much concern for this age-old territorial contest, but she might be interested to learn that the fallout from Hispanism’s quest for the “first modern novel” has involved so much attention to, indeed complication of, Don Quixote, that the book now resembles more a postmodern text than an early modern one. At the turn of the century, we have been left with what Jorge Luis Borges would recognize as an “aleph”: an infinite ideological labyrinth that reflects and/or cannibalizes all forms, thereby escaping all attempts [End Page 68] to describe it. 2 Whatever we currently mean by “Cervantes” (an author, a collection of texts, an ideological construction, and so forth) carries with it an impressive range of critical responses. Cervantes has been labeled converso (Castro, Canavaggio), Christian humanist (Castro, Bataillon, Forcione, Herrero, Vilanova), disillusioned secularist (Lukács, Cascardi), precapitalist (Johnson), anti-essentialist (Wilson), anti-Eusebian (Presberg), Menippean (Bakhtin), feminist (El Saffar, Rabin, Cruz), sadist (Nabokov), ethnocentric imperialist (Mariscal), medieval (Gorfkle), homophobic (Martín), non-organicist Aristotelian (Read), and either discursively or actually homosexual (Combet, Rossi, Smith, Arrabal). This list is nowhere near complete, but the reader will grasp the robust effects of the plurality of perspectives on Cervantes.Each of these interpretations is valid to varying degrees within various contexts, but at present I am interested in reading Cervantes as the author of a multicultural manifesto on behalf of the Moriscos of Southern Spain. 3 Within the contexts of French Marxist philosopher Louis Althusser and Palestinian postcolonial literary critic Edward Said, materialist, postcolonial, and multicultural critiques of various forms of power and their ideologies can be seen as the fundamental propositions of Cervantes’s Don Quixote. 4 Cervantes’s ultimate orientation may perhaps be inescapably Eurocentric, but his responses to the European experience of the rise and expansion of an ethnocentric militaristic nation state are relatively centrifugal when compared to the attitudes of many of his contemporaries.In surveying some of the antihegemonic details of Cervantes’s novel, I am also interested in dispelling the popular myth of Don Quixote. These notes of G. E. Moore's lectures for the three terms of 1934-1935 were compiled by Alice Ambrose, then Student of Newnham College, and Margaret Macdonald, then Fellow of Girton College. The lectures cover a wealth of interrelated topics, and provide instances of the analyses which made Moore «the father of the analytic school.» Since his analyses of such concepts as material objects, sense data, and truth rest on the ordinary use of expressions for these concepts, «ordinary language» philosophers found (...) in him their champion. This volume of Teilhardiana contains twenty-one essays and notes written between 1921 and 1955, some of which Teilhard published in journals and some of which were not published until after his death in 1955. Though the essays are on different topics, the constantly recurring theme is Teilhard's defense of evolution, or, more specifically, transformism, as a necessary framework principle for biological science, and in particular for that offshoot science of biology which Teilhard regards as the natural history of the origin (...) and development of thought, or the science of the phenomenon of man. Teilhard is willing to extend his genetic and teleologically informed method below the level of the life sciences to geology and eventually to the whole material-cum-radial-energy universe. At the same time as he is trying to overcome the objections of his fellow scientists to transformism on the basis of the paleontological record, Teilhard is constantly arguing the non-opposition between his evolutionism and the Christian doctrine of Creation. These are all familiar Chardinian themes, but what one gets in this volume that is lacking to a large extent even in The Phenomenon of Man is a more detailed account of Teilhard's philosophy of science. The critics have demanded this of Teilhard, and though it takes some piecing together, one can, on the basis of The Vision of the Past, sketch a portrait of Teilhard the critic of positivism and, particularly, instrumentalism. McDermott comes out of the long American tradition that takes the aim of philosophical inquiry to be interpretation of the open meanings of experience, so that we might all live fuller and richer lives. Here, the authors of these nine essays explore his highly original interpretations of philosophy's various questions about our shared existence. How are we to understand the nature of American culture and to carry forward its important contributions? What is the personal importance of (...) embodiment, of living in the realization of death? How does our physical and personal environment nourish bodies and spirits? What does the deliberate pursuit of a morality offer us? How can we carry forward the fundamental tasks of education to enable those who follow us to use our shared past to address their civic and spiritual problems? What are the possibilities for community? Together, these essays offer a clear, multi-layered understanding of the compelling vision that McDermott has presented over the years. In an Afterword, McDermott responds to the authors' queries and concerns, offering a restatement of his understanding of the American philosopher's task. These essays indicate, and McDermott's response confirms, that for him philosophy is not a purely cerebral activity. Philosophy is, rather, an intellectual means of exploring the fullness of human experience, and it functions best when it operates in the context of the broad sweep of the humanities. Similarly, for McDermott the self is no given substantial entity. On the contrary, it is relational, rooted geographically and socially in its place and its fellows, and damaged when these life-giving processes fail. Further, McDermott does not accept any ultimate canopy of meaning. The human journey is a personal project within which provisional meanings must be created to sustain our advance. (shrink)

My schedule appeared lighter than usual and I even envisioned leaving work a bit early. Maybe a challenging bike ride before dinner. I was sitting in the chairman’s office having our weekly meeting. One of our junior faculty members called... he needed help. He was on call and a 32–year–old pregnant woman (...) with a displaced femoral neck fracture had just been transferred to our emergency department (ED). Ms. B tripped in her kitchen last evening. She was unable to stand and was brought to the local ED by ambulance.My colleague was concerned that he wouldn’t be able to successfully reduce the fracture. The long–term prognosis of the hip joint depended on a successful reduction of the femoral neck fracture. In addition, there was an ongoing debate in the orthopaedic literature related to the urgency of operative care to prevent osteonecrosis of the femoral head. I reviewed the x–rays, didn’t anticipate a problem and let him know that I would come to the operating room (OR) to assist. “No”, he responded, “I am not comfortable caring for this patient”. I immediately left the office, went to the hospital and assumed care of the patient. Clearly, my plans for a bike ride were gone, but I had the opportunity to help a woman in need of expert orthopaedic care.Ms. B had been admitted to the obstetrical service. She was a 32–year–old woman of Haitian descent who had recently immigrated to the United States. She had very limited English comprehension and her primary spoken languages were French and Creole. She was alone, in a hospital in a foreign country. She was 24 weeks pregnant with her first child and had no other co–morbidities. Her pregnancy had ben progressing uneventfully.When I first met Ms. B she was lying on a stretcher in an observation unit on the obstetrical floor. She appeared frightened. Two orthopaedic residents were present and obtaining consent for surgical repair of the hip. In their minds there was nothing to discuss, the patient had a displaced femoral neck fracture and needed to have surgery immediately. In their minds, no other options were available and obtaining consent would be straightforward. I introduced myself, sat down on a lab stool by the bed and began a conversation. I needed to meet the patient, understand who she was and discuss treatment. I am not sure that obtaining an informed consent is ever “straightforward” when [End Page 14] a doctor is explaining to a patient that they require emergency surgery. This discussion surely would not be easy or straightforward.We utilized a telephone interpretation service. Every question and response required the transfer of the handset between the patient and physician. Ms. B was in pain and frightened. She immediately expressed concern about her pregnancy and her baby. I arranged for her to meet all of the physicians who would possibly be involved in her care and the care of her baby. Each consultant could explain their area of expertise and potential involvement in the care of Ms. B and her child. She met and had an opportunity to talk with the obstetrician on call, a maternal fetal medicine (MFM) fellow and a neonatologist. I supplied the orthopaedic information. I explained the severity of the injury, the risks of osteonecrosis either with or without surgery as well as concerns over non–union and traumatic arthritis of the hip. Undoubtedly, in a healthy 32–year–old woman who wasn’t pregnant, early surgical intervention would be the most appropriate management in hopes of preserving a normal hip joint. We also discussed the possibility of treating her without surgery. She would be administered pain medication, which were known to be safe during pregnancy, and allowed to begin to ambulate with a walker or crutches. I explained potential problems without surgery and what options would be available to treat any problems that developed with her hip after the child was born. Having rerud McCool's two books, I still and with a strong sense of gratitude articulate my philosophical self-des,ignation as Thomisit, but what I mean by Thomist has become considerably more nuanced.
